КЛИНИКО-БИОХИМИЧЕСКИЕ И ГЕМОСТАТИЧЕСКИЕ ОСОБЕННОСТИ ПАЦИЕНТОВ С САХАРНЫМ ДИАБЕТОМ 2 ТИПА ПРИ ХРОНИЧЕСКОЙ БОЛЕЗНИ ПОЧЕК 1–2 СТАДИЙ

Аннотация
Аннотация. Актуальность. Диабетическая болезнь почек является одним из наиболее распространённых осложнений сахарного диабета 2 типа и ведущей причиной хронической почечной недостаточности.
Цель исследования. Сравнить клинические, биохимические и коагуляционные показатели у пациентов с сахарным диабетом 2 типа и хронической болезнью почек (ХБП) 1 и 2 стадий.
Материалы и методы. В исследование включены 60 пациентов с СД 2 типа: 30 пациентов с ХБП 1 стадии и 30 пациентов с ХБП 2 стадии. Оценивали показатели функции почек, углеводного и липидного обмена, а также параметры системы гемостаза.
Результаты. Пациенты с ХБП 2 стадии были старше и имели большую продолжительность заболевания. У них выявлены более высокие уровни креатинина, мочевины, общего холестерина и триглицеридов по сравнению с пациентами ХБП 1 стадии. Изменения системы гемостаза характеризовались повышением уровня фибриногена, международного нормализованного отношения и протромбинового индекса при одновременном снижении активированного частичного тромбопластинового времени. Полученные данные свидетельствуют о формировании протромботического состояния уже на ранних стадиях ХБП.
Заключение. Прогрессирование ХБП у пациентов с сахарным диабетом 2 типа сопровождается ухудшением функции почек, нарастанием дислипидемии и нарушениями системы гемостаза, что требует ранней диагностики и комплексной профилактической коррекции.

Annotatsiya. Dolzarbligi. Diabetik buyrak kasalligi (DBK) 2-tip qandli diabetning (2-TQD) eng muhim mikrotomir asoratlaridan biri bo‘lib, terminal buyrak yetishmovchiligining asosiy sabablaridan hisoblanadi. KDIGO 2024 va ADA 2025 tavsiyalariga ko‘ra, hatto surunkali buyrak kasalligining (SBK) dastlabki bosqichlari ham yurak-qon tomir va buyrak asoratlari xavfining ortishi bilan bog‘liq bo‘lib, kasallikni erta aniqlash va davolash choralarini o‘z vaqtida boshlashni talab qiladi.
Tadqiqot maqsadi. 2-tip qandli diabet fonida rivojlangan SBKning 1 va 2 bosqichlaridagi bemorlarda klinik, biokimyoviy va koagulyatsion ko‘rsatkichlarni taqqoslash.
Materiallar va usullar. Tadqiqotga 2-tip qandli diabet va SBK 1-bosqichi (n=30) hamda SBK 2-bosqichi (n=30) bo‘lgan bemorlar kiritildi. Demografik ko‘rsatkichlar, uglevod va lipid almashinuvi, buyrak funksiyasi hamda gemostaz tizimi parametrlari baholandi.
Natijalar. SBK 2-bosqichidagi bemorlar yoshi kattaroq va kasallik davomiyligi uzoqroq bo‘lgan. Ularda kreatinin, mochevina, umumiy xolesterin va triglitseridlar darajasi yuqoriroq aniqlangan. Gemostaz tizimidagi o‘zgarishlar fibrinogen darajasining oshishi, protrombin indeksining ko‘payishi va faollashtirilgan qisman tromboplastin vaqtining qisqarishi bilan tavsiflangan. Olingan ma’lumotlar SBKning dastlabki bosqichlaridayoq protrombotik holat shakllanishini ko‘rsatadi.
Xulosa. 2-tip qandli diabet bilan og‘rigan bemorlarda SBK rivojlanishi buyrak funksiyasining pasayishi, dislipidemiya va gemostaz tizimi buzilishlari bilan kechadi, bu esa erta diagnostika va kompleks profilaktik choralarni talab qiladi.

Abstract. Background. Diabetic kidney disease (DKD) is one of the most important microvascular complications of type 2 diabetes mellitus (T2DM) and a leading cause of end-stage kidney disease. According to the KDIGO 2024 and ADA 2025 guidelines, even early stages of chronic kidney disease (CKD) are associated with increased cardiovascular and renal risks, highlighting the need for timely diagnosis and early intervention.
Objective. To compare clinical, biochemical, and coagulation parameters in patients with type 2 diabetes mellitus and stage 1–2 chronic kidney disease.
Materials and Methods. The study included patients with T2DM and CKD stage 1 (n=30) and CKD stage 2 (n=30). Demographic characteristics, carbohydrate and lipid metabolism parameters, kidney function indices, and hemostatic markers were assessed.
Results. Patients with CKD stage 2 were older and had a longer duration of diabetes. Higher levels of serum creatinine, urea, total cholesterol, and triglycerides were observed compared with patients with CKD stage 1. Hemostatic alterations were characterized by increased fibrinogen and prothrombin index levels together with shortened activated partial thromboplastin time. These findings indicate the development of a prothrombotic state even in the early stages of CKD.
Conclusion. Progression of CKD in patients with type 2 diabetes mellitus is associated with impaired kidney function, worsening dyslipidemia, and hemostatic disturbances, emphasizing the importance of early diagnosis and comprehensive preventive management.
