Rezyume. Dolzarbligi. Operatsiyadan keyingi ko‘ngil aynishi va qayt qilish (OKAQ) morbid semizlik bilan og‘rigan va bariatrik aralashuvlardan o‘tgan bemorlarda erta operatsion davrdagi eng tez-tez uchraydigan va klinik jihatdan muhim asoratlardan biridir. Ko‘plab prognostik shkalalar mavjudligiga qaramay, ularni bariatrik jarrohligida qo‘llash o‘ziga xos patofiziologik va jarrohlik xavf omillarining hisobga olinmaganligi tufayli cheklangan, bu esa ixtisoslashtirilgan prognozlash modellarini yaratish zaruriyatini keltirib chiqaradi.
Maqsad. Morbid semizlik bilan og‘rigan va laparoskopik bariatrik operatsiyalardan o‘tgan bemorlarda OKAQ rivojlanish xavfini individual baholash uchun yangi ixtisoslashtirilgan prognostik B-PONV Score shkalasini ishlab chiqish va klinik tadqiqotlarni o‘tkazish, shuningdek, uning samaradorligini an’anaviy shkalalar bilan solishtirganda baholash.
Materiallar va usullar. Tadqiqot ikki bosqichdan iborat bo‘ldi: xavf omillarining retrospektiv tahlili (n=106) va undan so‘ng shkalani ishlab chiqish hamda uning prospektiv klinik tadqiqoti. Nazorat guruhi bemorlari (n=53) standart ikki komponentli antiemetik profilaktika (deksametazon + ondansetron) olgan bo‘lsa, asosiy guruh bemorlari (n=53) droperidol qo‘shilgan xavfga asoslangan uch komponentli sxema bilan davolandi. Statistik ishlov berish logistik regressiya tahlili, ROC-tahlil (AUC, sezgirlik va o‘ziga xoslikni hisoblash)ni o‘z ichiga oldi.
Natijalar. 10 ta mustaqil prediktorni o‘z ichiga olgan B-PONV Score shkalasi ishlab chiqildi. Bu yuqori prognostik aniqlikni ko‘rsatdi: AUC 0,81 (95% CI: 0,73–0,89), sezgirlik — 78,4%, o‘ziga xoslik — 76,9% ni tashkil etdi. Apfel shkalasi uchun AUC ko‘rsatkichi sezilarli darajada past bo‘ldi — 0,71 (p<0,05). Xavfga asoslangan yondashuvni qo‘llash yuqori xavf guruhidagi bemorlarda OKAQ chastotasini 46,7% dan 12,3% gacha kamaytirishga (p<0,001) va zaxira terapiyasiga bo‘lgan ehtiyojni 41,5% ga qisqartirishga imkon berdi.
Xulosa. B-PONV Score shkalasi bariatrik jarrohlikda OKAQni shaxsiylashtirilgan prognozlash uchun samarali va klinik jihatdan qo‘llanilishi mumkin bo‘lgan vositadir. Undan foydalanish antiemetik profilaktika strategiyasini optimallashtirish, bemorlarning tiklanish sifatini yaxshilash imkonini beradi va buni keng joriy etish uchun tavsiya etilishi mumkin.

Summary. Background. Postoperative nausea and vomiting (PONV) are among the most common and clinically significant complications in the early postoperative period in patients with morbid obesity undergoing bariatric surgery. Despite the availability of multiple predictive scores, their application in the bariatric population is limited due to the lack of consideration for specific pathophysiological and surgical risk factors, necessitating the development of specialized predictive models.
Objective. To develop and clinically validate a new specialized predictive scale, the B-PONV Score, for individualized assessment of PONV risk in patients with morbid obesity after laparoscopic bariatric surgery, and to evaluate its effectiveness in comparison with traditional scales.
Materials and Methods. The study consisted of two phases: a retrospective analysis of risk factors (n=106) followed by scale development and its prospective clinical validation. Patients in the control group (n=53) received standard two-component antiemetic prophylaxis (dexamethasone + ondansetron), while the main group (n=53) received a risk-oriented three-component regimen with the addition of droperidol. Statistical analysis included logistic regression analysis, ROC analysis with calculation of AUC, sensitivity, and specificity.
Results. The B-PONV Score scale, including 10 independent predictors, was developed. Its validation demonstrated high predictive accuracy: AUC was 0.81 (95% CI: 0.73–0.89), sensitivity — 78.4%, specificity — 76.9%. The AUC value for the Apfel score was significantly lower — 0.71 (p<0.05). The application of the risk-oriented approach reduced the incidence of PONV in high-risk patients from 46.7% to 12.3% (p<0.001) and decreased the need for rescue therapy by 41.5%.
Conclusion. The B-PONV Score is an effective and clinically applicable tool for personalized prediction of PONV in bariatric surgery. Its use allows optimization of antiemetic prophylaxis strategies, improves the quality of patient recovery, and can be recommended for widespread implementation after external multicenter validation.
