Abstract. Medication-related osteonecrosis of the jaw (MRONJ) is characterized by pronounced structural heterogeneity of bone tissue, the quantitative assessment of which may have important diagnostic and clinical significance.
The aim of this study was to identify the most informative radiomic features of internal bone tissue heterogeneity in MRONJ and to evaluate their associations with the morphological and functional manifestations of the disease. A secondary analysis of radiomic features extracted from pathological lesions on computed tomography images was performed using the PyRadiomics software package. Entropy-based texture features demonstrated the highest diagnostic performance: the AUC was 0.91 for GLSZM Zone Entropy, 0.89 for GLCM Entropy, and 0.87 for GLCM Contrast. Significant correlations were identified between radiomic parameters and disease stage, lesion volume and area, number of sequestra, and the degree of functional limitation of mouth opening. The findings indicate that radiomic analysis enables objective and quantitative characterization of the internal heterogeneity of affected bone tissue, complements conventional visual assessment of computed tomography images, and expands the possibilities for comprehensive evaluation of the severity and extent of MRONJ.
