GMFCS III–V.
Annotatsiya. Tadqiqot maqsadi. GMFCS III–V darajali serebral falajli bolalarda chanoq-son bo‘g‘imining bir bosqichli rekonstruksiyasidan keyingi rentgenometrik dinamika va asoratlar profilini baholash.
Materiallar va usullar. 2020–2025 yillarda operatsiya qilingan 57 bemor (80 bo‘g‘im) retrospektiv tahlil qilindi. Barcha bemorlarga son suyagining varizatsiyalovchi derotatsion osteotomiyasi atsetabuloplastika bilan birgalikda bajarildi: Pemberton bo‘yicha — 42 bemorga (58 bo‘g‘im), Dega bo‘yicha — 15 bemorga (22 bo‘g‘im).
Natijalar. Reimers migratsiya indeksi 84,3 ± 15,2% dan 7,0 ± 2,5% gacha kamaydi va so‘nggi tekshiruvda 15,0 ± 10,1% ni tashkil etdi. Viberg burchagi −16,5 ± 5,4° dan 24,3 ± 11,5° gacha o‘zgardi; har ikkala ko‘rsatkich dinamikasi statistik ahamiyatli (p < 0,001). Avaskulyar nekroz 4 bo‘g‘imda (5,0%) rivojlandi.
Xulosa. Bir bosqichli rekonstruksiya son suyagi boshchasining qoplamasini yaxshiladi va o‘rtacha 3,1 yillik kuzatuvda korreksiyaning asosiy qismini saqlab qoldi.
Kalit so‘zlar. serebral falaj, son chiqishi, Reimers migratsiya indeksi, varizatsiyalovchi derotatsion osteotomiya, atsetabuloplastika, Pemberton osteotomiyasi, Dega osteotomiyasi, GMFCS.
Abstract. Purpose of the study. To assess radiographic changes and complications after one-stage hip reconstruction in children with cerebral palsy at GMFCS levels III–V.
Materials and methods. Fifty-seven patients (80 hips) operated on between 2020 and 2025 were reviewed retrospectively. All underwent femoral varus derotation osteotomy combined with acetabuloplasty: Pemberton in 42 patients (58 hips) and Dega in 15 patients (22 hips).
Results. The Reimers migration percentage decreased from 84.3 ± 15.2% to 7.0 ± 2.5% and was 15.0 ± 10.1% at final follow-up. The lateral centre-edge angle changed from −16.5 ± 5.4° to 24.3 ± 11.5°; both changes were statistically significant (p < 0.001). Avascular necrosis occurred in 4 hips (5.0%).
Conclusion. One-stage reconstruction improved femoral head coverage and maintained most of the correction at a mean follow-up of 3.1 years. Selection of acetabuloplasty according to triradiate cartilage status provides an age-adapted algorithm.
Key words. cerebral palsy, hip dislocation, Reimers migration percentage, varus derotation osteotomy, acetabuloplasty, Pemberton osteotomy, Dega osteotomy, GMFCS.
