Аннотация
Annotatsiya. Maqsad. Keksa yoshdagi bemorlarda yoshga bog‘liq giperkifoz va mushak-fassial rigidlikni fenotipga asoslangan personallashtirilgan konservativ korreksiya qilish dasturining klinik-biomexanik samaradorligi va xavfsizligini baholash.
Material va usullar. Prospektiv, parallel guruhli, randomizatsiyalangan nazoratli tadqiqotga 180 nafar bemor kiritilib, 60 nafardan uch guruhga taqsimlandi. I guruh fenotipga asoslangan 12 haftalik ko‘p komponentli dastur, II guruh yumshoq to‘qimalar terapiyasi bilan standart kineziterapiya, III guruh standart reabilitatsiya oldi. Asosiy natijalar Cobb burchagi va instrumental mushak-fassial rigidlik indeksi dinamikasi bo‘ldi. Qo‘shimcha natijalar og‘riqning raqamli reyting shkalasi (NRS), Oswestry Disability Index (ODI), Timed Up and Go (TUG), 10 metrga yurish tezligi, Berg Balance Scale (BBS), Falls Efficacy Scale-International (FES-I), 6 daqiqalik yurish testi, 6 oylik yiqilishlar va noxush holatlarni qamrab oldi.
Natijalar. I guruhda Cobb burchagi 53,8±9,0° dan 46,2±8,3° gacha kamaydi; o‘zgarish I va III guruhlar o‘rtasida 4,9° ni tashkil etdi (95% ishonch oralig‘i 3,4–6,4; p<0,001). Rigidlik indeksi I guruhda 1,08±0,23 dan 0,64±0,16 gacha pasaydi. TUG 12,8±3,0 soniyadan 10,4±2,5 soniyagacha qisqardi, yurish tezligi 0,82±0,18 dan 0,99±0,20 m/s gacha, BBS esa 43,0±6,3 dan 50,2±5,4 ballgacha yaxshilandi. Oldindan belgilangan klinik javob I guruhda 71,7%, II guruhda 48,3%, III guruhda 30,0% bemorda qayd etildi (umumiy p<0,001). Olti oyda yiqilgan bemorlar ulushi mos ravishda 11,7%, 20,0% va 28,3% bo‘ldi (uch guruhli umumiy p=0,074); I va III guruhlarning eksplorativ juft taqqoslashida RR=0,41 (95% ishonch oralig‘i 0,18–0,92; p=0,039). Jiddiy noxush holatlar kuzatilmadi.
Xulosa. Fenotipga asoslangan, yumshoq to‘qimalarni tayyorlash, maqsadli kineziterapiya, muvozanat mashqlari va uy dasturini birlashtirgan kompleks yondashuv yoshga bog‘liq giperkifozda postural geometriya, og‘riq va funksional ko‘rsatkichlarni standart reabilitatsiyaga nisbatan yaxshiladi. Yiqilishlar bo‘yicha natijalar istiqbolli bo‘lsa-da, ularni kattaroq tanlanmada tasdiqlash zarur.
Kalit so‘zlar: yoshga bog‘liq giperkifoz; mushak-fassial rigidlik; sagittal muvozanat; kineziterapiya; yiqilish profilaktikasi; keksa bemor; randomizatsiyalangan tadqiqot.

Abstract. Objective. To evaluate the clinical, biomechanical, and safety outcomes of a phenotype-based personalized conservative correction programme for age-related hyperkyphosis and myofascial rigidity in older patients.
Materials and methods. This prospective parallel-group randomized controlled trial included 180 patients allocated to three groups (60 per group). Group I received a 12-week phenotype-based multicomponent programme; Group II received standard kinesitherapy combined with soft-tissue therapy; and Group III received standard rehabilitation. The primary outcomes were changes in the Cobb angle and an instrument-based myofascial rigidity index. Secondary outcomes included the Numeric Rating Scale (NRS), Oswestry Disability Index (ODI), Timed Up and Go (TUG), 10-m gait speed, Berg Balance Scale (BBS), Falls Efficacy Scale-International (FES-I), 6-minute walk test, 6-month falls, adherence, and adverse events.
Results. In Group I, the Cobb angle decreased from 53.8±9.0° to 46.2±8.3°. The between-group difference in change between Groups I and III was 4.9° (95% CI 3.4–6.4; p<0.001). The rigidity index decreased from 1.08±0.23 to 0.64±0.16. TUG improved from 12.8±3.0 to 10.4±2.5 s, gait speed from 0.82±0.18 to 0.99±0.20 m/s, and BBS from 43.0±6.3 to 50.2±5.4 points. The prespecified clinical response was observed in 71.7%, 48.3%, and 30.0% of participants, respectively (overall p<0.001). At 6 months, 11.7%, 20.0%, and 28.3% of patients experienced at least one fall (overall p=0.074). In an exploratory pairwise comparison of Groups I and III, RR was 0.41 (95% CI 0.18–0.92; p=0.039). No serious intervention-related adverse events were recorded.
Conclusion. A phenotype-based programme combining soft-tissue preparation, targeted kinesitherapy, balance training, and a home programme improved postural geometry, pain, and functional outcomes compared with standard rehabilitation. The findings regarding falls are promising but require confirmation in a larger adequately powered trial.
