КЛИНИКО-ГЕНЕТИЧЕСКИЕ АССОЦИАЦИИ ПОЛИМОРФИЗМА ГЕНА MTHFR C677T И ПОКАЗАТЕЛЕЙ ОДНОУГЛЕРОДНОГО МЕТАБОЛИЗМА У ПАЦИЕНТОВ С АУТОИММУННЫМ ТИРЕОИДИТОМ

Аннотация
Аннотация. Аутоиммунный тиреоидит является одним из наиболее распространённых аутоиммунных заболеваний эндокринной системы. Полиморфизм гена MTHFR C677T ассоциирован со снижением активности фермента метилентетрагидрофолатредуктазы (MTHFR), что способствует накоплению гомоцистеина и развитию оксидативного стресса. Целью исследования явилось изучение особенности фолатного обмена и распространенность полиморфизма MTHFR C677T у пациентов с АИТ. В исследование были включены 19 пациентов с АИТ. Всем обследуемым проводились клинические, лабораторные и молекулярно-генетические исследования: ТТГ, свТ4, свТ3, АТ к тиреопероксидазе, АТ к тиреоглобулину , ГМЦ, витамина B12, фолиевой кислоты, витамина D и липидного спектра. Генотипирование MTHFR C677T выполнялось методом PCR-RFLP. Средний уровень ГМЦ составил 15,2±2,8 мкмоль/л, что соответствовало гипергомоцистеинемии. Генотипы распределились следующим образом: CC — 52,7%, CT — 36,8%, TT — 10,5%. У обследованных пациентов также выявлены недостаточность витамина D, дислипидемия и высокая частота избыточной массы тела. Полученные результаты свидетельствуют о возможной роли нарушений фолатного обмена и полиморфизма MTHFR C677T в патогенезе аутоимунного тиреоидита.

Annotatsiya. Autoimmun tireoidit endokrin tizimning eng keng tarqalgan autoimmun kasalliklaridan biri hisoblanadi. MTHFR C677T geni polimorfizmi metilentetragidrofolatreduktaza (MTHFR) fermenti faolligining pasayishi bilan bog‘liq bo‘lib, bu esa gomosisteinning to‘planishiga va oksidativ stress rivojlanishiga sabab bo‘ladi. Tadqiqotning maqsadi autoimmun tireoidit (AIT) bilan og‘rigan bemorlarda folat almashinuvi xususiyatlari hamda MTHFR C677T polimorfizmining tarqalishini o‘rganishdan iborat bo‘ldi. Tadqiqotga AIT tashxisi qo‘yilgan 19 nafar bemor jalb qilindi. Barcha ishtirokchilarda klinik, laborator va molekulyar-genetik tekshiruvlar o‘tkazildi: tireotrop gormon (TTG), erkin tiroksin (T4er), erkin triyodtironin (T3er), tireoperoksidazaga qarshi antitanalar, tireoglobulinga qarshi antitanalar, gomosistein (GMTS), B12 vitamini, foliy kislotasi, D vitamini hamda lipid spektri ko‘rsatkichlari aniqlandi. MTHFR C677T genotiplash PCR-RFLP usuli yordamida amalga oshirildi. O‘rtacha gomosistein darajasi 15,2±2,8 mkmol/l ni tashkil etib, bu giperhomosisteinemiya holatiga mos keldi. Genotiplar quyidagicha taqsimlandi: CC – 52,7 %, CT – 36,8 %, TT – 10,5 %. Tekshirilgan bemorlarda, shuningdek, D vitamini yetishmovchiligi, dislipidemiya va ortiqcha tana vaznining yuqori uchrash chastotasi aniqlandi. Olingan natijalar folat almashinuvi buzilishlari hamda MTHFR C677T polimorfizmi autoimmun tireoidit patogenezida muhim rol o‘ynashi mumkinligini ko‘rsatadi.

Abstract. Autoimmune thyroiditis (AIT) is one of the most common autoimmune disorders of the endocrine system. The MTHFR C677T gene polymorphism is associated with reduced activity of the methylenetetrahydrofolate reductase (MTHFR) enzyme, which contributes to homocysteine accumulation and the development of oxidative stress. The aim of this study was to investigate the characteristics of folate metabolism and the prevalence of the MTHFR C677T polymorphism in patients with AIT. The study included 19 patients diagnosed with autoimmune thyroiditis. All participants underwent clinical, laboratory, and molecular genetic assessments, including measurements of thyroid-stimulating hormone (TSH), free thyroxine (fT4), free triiodothyronine (fT3), anti-thyroid peroxidase antibodies (anti-TPO), anti-thyroglobulin antibodies (anti-Tg), homocysteine (Hcy), vitamin B12, folic acid, vitamin D, and lipid profile parameters. Genotyping of the MTHFR C677T polymorphism was performed using the polymerase chain reaction–restriction fragment length polymorphism (PCR-RFLP) method. The mean homocysteine level was 15.2 ± 2.8 μmol/L, corresponding to hyperhomocysteinemia. The distribution of genotypes was as follows: CC – 52.7%, CT – 36.8%, and TT – 10.5%. In addition, vitamin D deficiency, dyslipidemia, and a high prevalence of overweight status were identified among the examined patients. The obtained results suggest a possible role of folate metabolism disorders and the MTHFR C677T polymorphism in the pathogenesis of autoimmune thyroiditis.
