КЛИНИКО-ПАТОГЕНЕТИЧЕСКАЯ ХАРАКТЕРИСТИКА НАРУШЕНИЙ СПЕРМАТОГЕНЕЗА, ГОРМОНАЛЬНО-МЕТАБОЛИЧЕСКИХ И ИНФЕКЦИОННЫХ ПОКАЗАТЕЛЕЙ У МУЖЧИН С ПЕРВИЧНЫМ ИДИОПАТИЧЕСКИМ БЕСПЛОДИЕМ В ЗАВИСИМОСТИ ОТ ИНДЕКСА МАССЫ ТЕЛА

Аннотация
Аннотация. Идиопатическое мужское бесплодие остаётся одной из наиболее сложных и недостаточно изученных проблем современной андрологии. Несмотря на отсутствие выраженной органической патологии, у значительной части пациентов выявляются функционально-метаболические нарушения, оказывающие негативное влияние на сперматогенез. Особое значение приобретает изучение роли избыточной массы тела как одного из факторов, способствующих развитию гормонального дисбаланса, оксидативного стресса и субклинических нарушений тестикулярной функции. Избыточная масса тела у мужчин с первичным идиопатическим бесплодием ассоциируется с более выраженными нарушениями сперматогенеза, изменениями гормонального профиля и снижением биохимической активности спермоплазмы. Полученные данные подтверждают значимую роль метаболических факторов в патогенезе репродуктивной дисфункции и свидетельствуют о необходимости комплексной оценки метаболического статуса у пациентов с идиопатическим мужским бесплодием.

Abstract. Idiopathic male infertility remains one of the most complex and poorly studied problems of modern andrology. Despite the absence of pronounced organic pathology, a significant part of patients have functional metabolic disorders that have a negative impact on spermatogenesis. Of particular importance is the study of the role of overweight as one of the factors contributing to the development of hormonal imbalances, oxidative stress and subclinical disorders of testicular function. Excess body weight in men with primary idiopathic infertility is associated with more pronounced disorders of spermatogenesis, changes in the hormonal profile and a decrease in the biochemical activity of spermoplasma. The findings confirm the significant role of metabolic factors in the pathogenesis of reproductive dysfunction and suggest the need for a comprehensive assessment of metabolic status in patients with idiopathic male infertility.

Izohlar. Idiopatik erkak bepushtligi zamonaviy andrologiyaning eng murakkab va yetarlicha oʻrganilmagan muammolaridan biri boʻlib qolmoqda. Aniqlangan organik patologiya yo’qligiga qaramay, bemorlarning katta qismida spermatogenezga salbiy ta’sir ko’rsatuvchi funksional-metabolik buzilishlar aniqlanadi. Gormonal nomutanosiblik, oksidativ stress va testikulyar funksiyaning subklinik buzilishi rivojlanishiga yordam beruvchi omillardan biri sifatida ortiqcha vaznning rolini oʻrganish alohida ahamiyat kasb etadi. Birlamchi idiopatik bepushtligi bo’lgan erkaklarda ortiqcha vazn spermatogenezning ko’proq namoyon bo’lgan buzilishi, gormonal profilning o’zgarishi va spermoplazmaning biokimyoviy faolligining pasayishi bilan bog’liq. Olingan ma’lumotlar reproduktiv disfunksiya patogenezida metabolik omillarning ahamiyatli rolini tasdiqlaydi va idiopatik erkak bepushtligi bilan og’rigan bemorlarda metabolik maqomni kompleks baholash zarurligidan dalolat beradi.
