КЛИНИЧЕСКОЕ ЗНАЧЕНИЕ НЕФРИНА СЫВОРОТКИ КРОВИ В РАННЕЙ ДИАГНОСТИКЕ ДИАБЕТИЧЕСКОЙ НЕФРОПАТИИ У ДЕТЕЙ И ПОДРОСТКОВ С САХАРНЫМ ДИАБЕТОМ 1 ТИПА

Аннотация
Аннотация. Диабетическая нефропатия является одним из наиболее тяжёлых микрососудистых осложнений сахарного диабета 1 типа и ведущей причиной хронической болезни почек. В исследование были включены 80 детей и подростков с сахарным диабетом 1 типа в стадии нормоальбуминурии и 16 здоровых детей. Определяли уровень нефрина в сыворотке крови методом иммуноферментного анализа. Установлено достоверное повышение концентрации нефрина у пациентов с сахарным диабетом 1 типа по сравнению с контрольной группой. Повышенный уровень нефрина выявлялся ещё до появления микроальбуминурии и ассоциировался с последующим ухудшением функции почек. Полученные результаты свидетельствуют о высокой диагностической и прогностической значимости нефрина как раннего биомаркера диабетической нефропатии у детей и подростков.

Annotatsiya. Diabetik nefropatiya 1-tip qandli diabetning eng og‘ir mikrotomir asoratlaridan biri bo‘lib, surunkali buyrak kasalligining asosiy sabablaridan hisoblanadi. Tadqiqotga normoalbuminuriya bosqichidagi 1-tip qandli diabet bilan kasallangan 80 nafar bolalar va o‘smirlar hamda 16 nafar sog‘lom bolalar kiritildi. Qon zardobidagi nefrin darajasi immunoferment tahlil (ELISA) usuli yordamida aniqlandi. 1-tip qandli diabet bilan kasallangan bemorlarda nefrin konsentratsiyasining nazorat guruhiga nisbatan ishonchli darajada yuqori ekanligi aniqlandi. Nefrin darajasining oshishi mikroalbuminuriya rivojlanishidan oldin aniqlanib, keyinchalik buyrak funksiyasining yomonlashuvi bilan bog‘liq ekanligi kuzatildi. Olingan natijalar nefrinning bolalar va o‘smirlarda diabetik nefropatiyani erta aniqlash uchun yuqori diagnostik va prognostik ahamiyatga ega biomarker ekanligini ko‘rsatadi.

Abstract. Diabetic nephropathy is one of the most severe microvascular complications of type 1 diabetes mellitus and a leading cause of chronic kidney disease. The study included 80 children and adolescents with type 1 diabetes mellitus at the normoalbuminuric stage and 16 healthy children. Serum nephrin levels were determined using an enzyme-linked immunosorbent assay (ELISA). A significantly increased serum nephrin concentration was detected in patients with type 1 diabetes mellitus compared with the control group. Elevated nephrin levels were identified before the onset of microalbuminuria and were associated with subsequent deterioration of renal function. The findings indicate the high diagnostic and prognostic value of nephrin as an early biomarker of diabetic nephropathy in children and adolescents.
