СРАВНИТЕЛЬНАЯ КЛИНИЧЕСКАЯ ЦЕННОСТЬ ЛИПИДНЫХ И АТЕРОГЕННЫХ ИНДЕКСОВ У ПАЦИЕНТОВ С СЕРДЕЧНО-СОСУДИСТЫМИ ЗАБОЛЕВАНИЯМИ И МЕТАБОЛИЧЕСКИМ СИНДРОМОМ: ФОКУС НА ОСТАТОЧНОМ СЕРДЕЧНО-СОСУДИСТОМ РИСКЕ

Аннотация
Аннотация. Актуальность. Сердечно-сосудистые заболевания атеросклеротического генеза остаются тесно связанными с нарушениями липидного обмена, при этом сочетание атеросклеротического сердечно-сосудистого заболевания (АСССЗ) с метаболическим синдромом характеризуется более сложным липопротеиновым фенотипом, включающим гипертриглицеридемию, снижение холестерина липопротеинов высокой плотности (ХС-ЛПВП), увеличение числа аполипопротеин-B-содержащих частиц и накопление ремнантных липопротеинов. Холестерин липопротеинов низкой плотности (ХС-ЛПНП) сохраняет статус основной терапевтической мишени, однако при инсулинорезистентности, ожирении, сахарном диабете и гипертриглицеридемии его концентрация не всегда адекватно отражает количество циркулирующих атерогенных частиц. В связи с этим особое значение приобретают холестерин не-ЛПВП (non-HDL-C), аполипопротеин B (ApoB), отношение ApoB/ApoA1, триглицериды/ХС-ЛПВП (ТГ/ХС-ЛПВП), атерогенный индекс плазмы (AIP), ремнантный холестерин и липидно-метаболический индекс TyG. Современные данные свидетельствуют, что ApoB и non-HDL-C особенно информативны при дискордантности с ХС-ЛПНП, тогда как ТГ/ХС-ЛПВП, AIP и TyG лучше отражают метаболический компонент атерогенной дислипидемии и инсулинорезистентности.
Цель настоящего обзора — провести сравнительный анализ традиционных и расчетных липидных индексов и определить их место в оценке остаточного сердечно-сосудистого риска у пациентов с сердечно-сосудистыми заболеваниями и метаболическим синдромом.

Annotatsiya. Dolzarbligi. Aterosklerotik genezli yurak-qon tomir kasalliklari lipid almashinuvi buzilishlari bilan chambarchas bog‘liq bo‘lib qolmoqda. Aterosklerotik yurak-qon tomir kasalligi (AYuQTK)ning metabolik sindrom bilan birga kechishi yanada murakkab lipoprotein fenotipi bilan tavsiflanadi; u gipertriglitseridemiya, yuqori zichlikdagi lipoproteinlar xolesterini (HDL-C) miqdorining pasayishi, apolipoprotein B saqlovchi zarrachalar sonining ortishi hamda remnant lipoproteinlarning to‘planishini o‘z ichiga oladi. Past zichlikdagi lipoproteinlar xolesterini (LDL-C) asosiy terapevtik nishon maqomini saqlab qolmoqda, biroq insulin rezistentligi, semizlik, qandli diabet va gipertriglitseridemiya mavjud bo‘lganda uning konsentratsiyasi qonda aylanib yuruvchi aterogen zarrachalar sonini har doim ham yetarli darajada aks ettirmaydi. Shu munosabat bilan no-HDL xolesterin (non-HDL-C), apolipoprotein B (ApoB), ApoB/ApoA1 nisbati, triglitseridlar/HDL-C (TG/HDL-C) nisbati, plazmaning aterogen indeksi (AIP), remnant xolesterin hamda lipid-metabolik TyG indeksi alohida ahamiyat kasb etadi. Zamonaviy ma’lumotlar ApoB va non-HDL-C ko‘rsatkichlari LDL-C bilan diskordantlik mavjud bo‘lganda ayniqsa informativ ekanligini ko‘rsatadi, TG/HDL-C, AIP va TyG esa aterogen dislipidemiyaning metabolik komponenti va insulin rezistentligini yaxshiroq aks ettiradi.
Ushbu sharhning maqsadi — an’anaviy va hisoblab chiqiladigan lipid indekslarini qiyosiy tahlil qilish hamda ularning yurak-qon tomir kasalliklari va metabolik sindromi bo‘lgan bemorlarda qoldiq yurak-qon tomir xavfini baholashdagi o‘rnini aniqlash.

Abstract. Background. Atherosclerotic cardiovascular diseases remain closely associated with disturbances in lipid metabolism, while the coexistence of atherosclerotic cardiovascular disease (ASCVD) and metabolic syndrome is characterized by a more complex lipoprotein phenotype, including hypertriglyceridemia, reduced high-density lipoprotein cholesterol (HDL-C), an increased number of apolipoprotein B-containing particles, and the accumulation of remnant lipoproteins. Low-density lipoprotein cholesterol (LDL-C) remains the primary therapeutic target; however, in the presence of insulin resistance, obesity, diabetes mellitus, and hypertriglyceridemia, its concentration does not always adequately reflect the number of circulating atherogenic particles. Therefore, non-high-density lipoprotein cholesterol (non-HDL-C), apolipoprotein B (ApoB), the ApoB/ApoA1 ratio, the triglyceride-to-HDL-C ratio (TG/HDL-C), the atherogenic index of plasma (AIP), remnant cholesterol, and the triglyceride-glucose (TyG) index have gained particular clinical relevance. Current evidence indicates that ApoB and non-HDL-C are particularly informative in cases of discordance with LDL-C, whereas TG/HDL-C, AIP, and TyG more accurately reflect the metabolic component of atherogenic dyslipidemia and insulin resistance.
Objective. This review aimed to comparatively analyze traditional and calculated lipid indices and to determine their role in the assessment of residual cardiovascular risk in patients with cardiovascular disease and metabolic syndrome.
