ОСОБЕННОСТИ МОРФОЛОГИЧЕСКОГО СТРОЕНИЯ СЛИЗИСТОЙ ОБОЛОЧКИ ПОЛОСТИ РТА И ЕЁ РОЛЬ В ПАТОГЕНЕЗЕ ХРОНИЧЕСКОГО РЕЦИДИВИРУЮЩЕГО АФТОЗНОГО СТОМАТИТА

Аннотация
Аннотация. Хронический рецидивирующий афтозный стоматит (ХРАС) остаётся одной из наиболее частых и одновременно недостаточно предсказуемых воспалительно-деструктивных патологий слизистой оболочки полости рта (СОПР). Несмотря на значительный объём клинических наблюдений, ключевым нерешённым вопросом остаётся объяснение того, почему афты преимущественно формируются на участках подвижной, неороговевающей слизистой оболочки, а рецидивы возникают даже при отсутствии выраженной микробной или травматической причины.
Цель настоящего обзора — систематизировать современные представления о морфологической организации СОПР и показать её значение в патогенезе ХРАС. На основании данных современных публикаций рассматриваются особенности многослойного плоского эпителия, базальной мембраны, собственной пластинки, микроциркуляторного русла, клеток врождённого и адаптивного иммунитета, слюнного барьера и микробиоценоза. Показано, что СОПР является не пассивной анатомической оболочкой, а активным иммунно-эпителиальным органом, где кератиноциты, дендритные клетки, Т-лимфоциты, нейтрофилы, макрофаги, цитокины и микробиота формируют единую систему поддержания гомеостаза. При ХРАС нарушение этой системы проявляется локальной эпителиальной дезорганизацией, повышением проницаемости барьера, периваскулярной лимфоцитарной инфильтрацией, цитотоксическим повреждением кератиноцитов и последующим образованием поверхностного некротического дефекта – афты. Морфологическая уязвимость неороговевающей слизистой оболочки, особенно слизистой губ, щёк, переходных складок и вентральной поверхности языка, объясняет типичную топографию поражения. Обзор подчёркивает необходимость комплексного клинико-морфологического подхода к диагностике ХРАС с учётом состояния эпителиального барьера, местного иммунитета, микробиоты, слюны и системных факторов пациента.

Annotatsiya. Surunkali qaytalanuvchi aftoz stomatit (SQAS) og‘iz bo‘shlig‘i shilliq qavatining (OBShQ) eng ko‘p uchraydigan, shu bilan birga klinik kechishi yetarli darajada oldindan bashorat qilinmaydigan yallig‘lanish-destruktiv kasalliklaridan biri bo‘lib qolmoqda. Ko‘plab klinik kuzatuvlar mavjud bo‘lishiga qaramay, aftalarning asosan harakatchan, muguzlanmaydigan shilliq qavat sohalarida shakllanishi hamda kasallik qaytalanishlarining aniq mikrob yoki travmatik omil bo‘lmagan holatlarda ham yuzaga kelishi hanuzgacha to‘liq izohlanmagan muhim muammo hisoblanadi.
Mazkur sharhning maqsadi — OBShQning morfologik tuzilishi haqidagi zamonaviy qarashlarni tizimlashtirish va uning SQAS patogenezidagi ahamiyatini ko‘rsatib berishdan iborat. Zamonaviy ilmiy manbalar ma’lumotlari asosida ko‘p qavatli yassi epiteliy, bazal membrana, xususiy plastinka, mikrosirkulyator o‘zan, tug‘ ma va adaptiv immunitet hujayralari, so‘lak to‘sig‘i hamda og‘iz mikrobiotsenozining o‘ziga xos xususiyatlari tahlil qilinadi. Ko‘rsatib o‘tilishicha, og‘iz bo‘shlig‘i shilliq qavati passiv anatomik qoplama emas, balki faol immun-epitelial a’zo bo‘lib, unda keratinotsitlar, dendrit hujayralar, T-limfotsitlar, neytrofillar, makrofaglar, sitokinlar va mikrobiota mahalliy gomeostazni ta’minlovchi yagona tizimni shakllantiradi. SQASda ushbu tizimning buzilishi mahalliy epitelial dezorganizatsiya, to‘siq o‘tkazuvchanligining oshishi, perivaskulyar limfotsitar infiltratsiya, keratinotsitlarning sitotoksik shikastlanishi va keyinchalik yuzaki nekrotik nuqson – afta hosil bo‘lishi bilan namoyon bo‘ladi. Ayniqsa lablar, lunjlar, o‘tish burmalari va tilning ventral yuzasi shilliq qavatiga xos bo‘lgan muguzlanmaydigan shilliq qavatning morfologik zaifligi zararlanishlarning odatiy topografiyasini tushuntirib beradi. Ushbu sharh SQAS diagnostikasida epitelial to‘siq holati, mahalliy immunitet, mikrobiota, so‘lak va bemorning tizimli omillarini hisobga olgan holda kompleks klinik-morfologik yondashuv zarurligini ta’kidlaydi.

Abstract. Chronic recurrent aphthous stomatitis (CRAS) remains one of the most common and, at the same time, insufficiently predictable inflammatory-destructive disorders of the oral mucosa. Despite the considerable amount of accumulated clinical observations, the key unresolved issue remains the explanation of why aphthae predominantly develop in areas of mobile, non-keratinized mucosa, and why recurrences occur even in the absence of an evident microbial or traumatic cause.
The aim of this review is to systematize current concepts regarding the morphological organization of the oral mucosa and to demonstrate its significance in the pathogenesis of CRAS. Based on data from contemporary publications, the review discusses the structural features of the stratified squamous epithelium, basement membrane, lamina propria, microcirculatory bed, cells of innate and adaptive immunity, salivary barrier, and oral microbiocenosis. It is shown that the oral mucosa is not a passive anatomical covering, but an active immune-epithelial organ in which keratinocytes, dendritic cells, T lymphocytes, neutrophils, macrophages, cytokines, and the microbiota form an integrated system responsible for maintaining local homeostasis. In CRAS, disruption of this system is manifested by local epithelial disorganization, increased barrier permeability, perivascular lymphocytic infiltration, cytotoxic damage to keratinocytes, and subsequent formation of a superficial necrotic defect – an aphtha. The morphological vulnerability of the non-keratinized mucosa, particularly the mucosa of the lips, cheeks, vestibular folds, and the ventral surface of the tongue, explains the typical topography of the lesions. This review emphasizes the need for an integrated clinical and morphological approach to the diagnosis of CRAS, taking into account the condition of the epithelial barrier, local immunity, microbiota, saliva, and systemic factors of the patient.
