РОЛЬ ВИТАМИНА D В ПАТОГЕНЕЗЕ И ТЕРАПИИ ПСОРИАЗА

Аннотация
Аннотация. В обзоре представлен анализ роли витамина D в патогенезе и терапии псориаза — системной иммуноопосредованной патологии, поражающей 2–3% мировой популяции. Описаны механизмы регуляции дифференцировки кератиноцитов и супрессии клеточных ответов Th1/Th17 метаболитами нутриента. На основе эпидемиологических данных показана высокая распространенность гиповитаминоза D среди пациентов (76–85%) и его обратная корреляция с индексом PASI. С позиций гипотезы «свободного гормона» обосновано преимущество оценки свободного 25(OH)D и витамин-D-связывающего белка как биомаркеров системного воспаления и псориатического артрита. Освещен вклад полиморфизмов гена рецептора витамина Д в восприимчивость к дерматозу. Рассмотрены топическое применение аналогов витамина D, эффекты фототерапии и дискуссионные аспекты пероральной саплиментации. Доказана связь дефицита активной формы 1,25(OH)2D с кардиометаболическими осложнениями и обоснована необходимость персонализированного мониторинга витаминного статуса в клинике.

Abstract. The review provides an analysis of the role of vitamin d in the pathogenesis and therapy of psoriasis, a systemic immune-mediated pathology affecting 2–3% of the global population. It describes the mechanisms of keratinocyte differentiation regulation and the suppression of Th1/Th17 cellular responses by nutrient metabolites. Based on epidemiological data, a high prevalence of hypovitaminosis d among patients (76–85%) and its inverse correlation with the PASI index are demonstrated. From the standpoint of the «free hormone» hypothesis, the advantage of evaluating free 25(OH)D and vitamin D-binding protein as biomarkers of systemic inflammation and psoriatic arthritis is substantiated. The contribution of vitamin D receptor gene polymorphisms to susceptibility to the dermatosis is highlighted. The topical use of vitamin d analogs, the effects of phototherapy, and the controversial aspects of oral supplementation are reviewed. The link between the deficiency of the active form 1,25(OH)2D and cardiometabolic complications is established, and the necessity of personalized monitoring of vitamin d status in clinical practice is justified.

Annotatsiya. Ushbu sharhda jahon aholisining 2–3 foizini qamrab olgan tizimli immun vositachiligidagi patologiya — psoriaz patogenezi va terapiyasida D vitaminining o‘rni tahlil qilingan. Nutriyent metabolitlari tomonidan keratinotsitlar differensiatsiyasini tartibga solish hamda Th1/Th17 hujayra javoblarini susaytirish mexanizmlari yoritib berilgan. Epidemiologik ma’lumotlarga ko‘ra, bemorlar orasida D gipovitaminozining yuqori darajada (76–85%) tarqalgani va uning PASI indeksi bilan teskari korrelyatsiyaga ega ekanligi ko‘rsatilgan. “Erkin gormon” gipotezasi nuqtayi nazaridan, tizimli yallig‘lanish va psoriatik artritning biomarkerlari sifatida erkin 25(OH)D va D vitaminini bog‘lovchi oqsilni baholashning afzalliklari asoslab berilgan. Vitamin D reseptori genining polimorfizmlari dermatozga moyillikda o‘z hissasiga ega ekani ochib berilgan. D vitamini analoglarini topikal qo‘llash, fototerapiya samaralari va peroral saplimentatsiyaning munozarali jihatlari ko‘rib chiqilgan. Faol shakldagi 1,25(OH)2D tanqisligining kardiometabolik asoratlar bilan bog‘liqligi isbotlangan va klinik amaliyotda bemorning vitamin statusini individual nazorat qilish zarurati asoslantirilgan.
